RESPONSIBILITIESย 

Direct Care:ย 

  • Administers or prescribes treatmentsย 
  • Discusses potential side effects to medications and/or immunizations with patientsย 
  • Promotes health by advising patients about diet, hygiene, and methods for disease preventionย 
  • Refers patients to medical specialists for consultant services for their well-being, as neededย 
  • Follows up with patients regarding progress in high risk or emergency casesย 
  • Conducts physical examinations to provide information needed for admission to schools, consideration for jobs, or eligibility for insurance coverageย 

Provider and Team Support:ย 

  • Shares accountability for overall patient health outcomes by working in coordination with care teamsย 
  • Operates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholdersย 
  • Assesses training needs and promotes developmental activities of medical staffย 

Transgender Patient Care:ย 

  • Provides competent and culturally sensitive primary care services to patientsย 
  • Offers hormone therapy management, including prescribing and monitoring hormone regimensย 
  • Addresses and manages the physical, emotional, and mental health needs of transgender individualsย 
  • Conducts assessments for gender affirming surgeries and facilitate appropriate referrals, as neededย 
  • Works with the behavioral health team by assisting with gender identity exploration and provides support throughout the transitioning processย 

HIV Patient Careย 

  • Offers specialized care to patients living with HIV, including antiretroviral therapy management.ย 
  • Monitors disease progression, immune statuses, and potential complications related to HIVย 
  • Provides counseling and education on adherence to treatment plans and preventative measuresย 
  • Collaborates with interdisciplinary teams to develop and implement comprehensive care plansย 

Clinical Administration:ย 

  • Reviews patient files/records, including allergies, problems, medications, and immunization statusย 
  • Elicits and records information about patient medical historyย 
  • Examines patients for symptoms or physical informationย 
  • Orders and executes various test, analyses, and diagnostic images to provideย 
  • information on patientโ€™s conditionย 
  • Analyzes reports and findings of tests, examinations, and diagnoses to understand the patientโ€™s current conditionย 
  • Determines and prescribes patient medications, dosages and schedules dependent on their conditions and allergiesย 
  • Prescribes and administers vaccinations to immunize patient from communicable diseasesย 
  • Documents patient visits, including medical history, physical exam, diagnoses, and plan of actionย 
  • Stays informed of organizational activities and promote mission and goalsย 
  • Reviews and responds to all forms of communication, such as emails, postal mail, voice mailsย 
  • Functions at highest level according to credentials and competencyย 
  • Impacts patient experience by demonstrating courteous and helpful behavior and a commitment to accuracyย 

Customer Relations:ย 

  • Maintains professional working relationships with all levels of staff, clients, and the publicย 
  • Cooperates in accomplishing department goals and objectivesย 

ย 

EDUCATION/EXPERIENCEย 

  • Doctorate in medicine requiredย 
  • Valid CA Medical license in good standing requiredย 
  • Valid DEA and CPR certifications requiredย 
  • WPATH Training requiredย 
  • AAHIVS HIV Specialist Certification requiredย 
  • Experience working with transgender patients and/or those living with HIV preferredย

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsย 
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule formย 
  • Knowledgeable about and experience with providing direct care to patientsย 
  • Ability to commit to diversity/inclusion and providing patient centered, non-discriminatory and culturally competent careย 
  • Knowledgeable about and experience with unique healthcare needs and challenges faced by transgender individuals and those living with HIVย 
  • Knowledgeable about and experience with gender affirming care and HIV treatment protocolsย 
  • Ability to successfully manage multiple tasks simultaneouslyย ย 
  • Excellent planning and organizational abilityย 
  • Ability to work as part of a team as well as independentlyย 
  • Ability to work with highly confidential information in a professional and ethical mannerย ย 

Physical Requirementsย 

  • Ability to lift/carry 20 lbs/weightย 
  • Ability to stand for long periods of timeย

Pay Range: $229,100-$384,900 per year, depending on experience.
