Job Summary:
The Transgender and Gender Expansive Case Manager provides patient-centered medical and psychosocial case management in support of individualized care plans. This role focuses on care coordination, prior authorizations, advocacy, and linkage to internal and external resources for transgender, gender expansive, and nonbinary patients.

Key Responsibilities:

  • Provide case management and care coordination for transgender and gender expansive patients

  • Coordinate prior authorizations for medications, including tracking, appeals, and collaboration with providers, pharmacy, and insurance

  • Develop psychosocial care plans in collaboration with medical and behavioral health teams

  • Identify patients in crisis and refer to Behavioral Health Clinicians as needed

  • Assist with surgical letters, name changes, and legal documentation support

  • Advocate for patients to ensure affirming, respectful, and responsive care

  • Maintain logs, documentation, and confidentiality in compliance with regulations

  • Build and maintain referral networks and community resources

  • Participate in team meetings, trainings, and interdisciplinary coordination

Qualifications:

  • Experience in medical and/or psychosocial case management

  • Experience working with transgender, gender expansive, and LGBTQ+ communities (required)

  • Knowledge of healthcare regulations, confidentiality, and community resources

  • Strong communication, organizational, and advocacy skills

  • Ability to manage a caseload and work collaboratively in a team environment

  • Bilingual (Spanish or other languages) preferred

Education & Experience:

  • Certified Medical Assistant preferred

  • Bachelor’s degree in Social Work or related field or 2 years of relevant experience

  • Case management and prior authorization experience preferred

Physical Requirements:

  • Ability to sit, stand, walk, and use hands frequently

  • Ability to lift up to 25 lbs occasionally
